Apply now to Focus Academy, a charter middle school, high school and transition program for students with special needs. If you have questions regarding the number of spots available for the current year, please contact the main office at 813.443.5558 for more information.

Email or Drop Off

Complete this fillable PDF version and email it to admissions@focusacademytampa.org or drop it off in the main office along with the following documents:

A copy of your students most recent IEP
Academic transcripts (grades 10 and above)
Most recent report card (grades 6-9)
Behavior Plan/FBA (if applicable)
Recent psychological evaluation (if available)
Recent photo of student

Apply Online

Submit an application by clicking the link and following the prompts to create an account in our student information system. Complete the form, upload supporting documents through the portal and click “Submit.” Once the application is received, you will be able to log back in to see where your application is in the review process. For your convenience, the online application is mobile-friendly.

All applications should include the following student information:

1. IEP or Individualized Education Plan (If currently enrolled in a private school, please send the last IEP that the student received)

2. Psychological evaluation/testing, if available

3. Current photo of student

Applications and supporting documentation can also be dropped off or mailed to Focus Academy, or faxed to 813.443.5630.
